1. Do you know the date, format, and purpose of the annual review meeting?
2. Have you read the current EHCP recently enough to know what it says?
3. Do you know whether the support written in the EHCP has actually been delivered?
4. Have you gathered recent examples of how your child is doing in school?
5. Do you know what is working well and what is not working well right now?
6. Have you identified any outcomes or provision that may need changing?
7. Have you written down the key points you want to raise in the meeting?
8. Do you have any recent reports, communication, or evidence you may want to refer to?
